Recognizing Signs and Symptoms
of Behavioral Health Changes
Which May Indicate an Illness

There are specific screening indicators to assist individuals, as well as health professionals, determine risk or potential behavioral health changes/illnesses such as clinical depression, dementia, and anxiety.

This list is not all inclusive and is not diagnostic in nature; it is simply a self-screening tool. Should you answer yes to more than 4 of these questions, you may have identified presenting issues or problems that you should discuss with your physician or a behavioral health professional.

  • Do you worry excessively about life issues that may or may not be in your control?
  • Do you find it extremely difficult to control your worries, to the point that it interferes with your ability to focus and get things done?
  • Do you have difficulty concentrating or experiencing irrational thought patterns?
  • Do you feel more irritable and tired than usual?
  • Have there been unexplained changes in your sleep patterns: sleeping more or less?
  • Do you feel more emotional than usual; excessive anger or crying?
  • Do you have bouts with uncontrollable fear for unknown reasons?
  • Have you experienced substantial weight gain or loss in a short period of time?
  • Have you lost interest in life experiences and relationships, isolating yourself from others and activities?
  • Do you experience episodes of feeling of helpless and hopeless?
  • Have you had thoughts of hurting yourself or others?
  • Are you suspicious of others, feeling they are against you and do not understand?
  • Do you have difficulty in overcoming grief, loss, or other life adjustments?
